I understand and agree that: The Authorized Parties may use all or part of my testimonial, with or without attribution, and may edit it for clarity, length, grammar, formatting, or marketing purposes, so long as the general meaning is not materially altered. The Authorized Parties are under no obligation to use my testimonial. I waive any right to inspect or approve the finished materials or specific uses of the Testimonial Materials. I represent that the testimonial reflects my honest opinions and actual experience. I confirm that I have the legal authority to provide this authorization and that doing so does not violate any employment agreement, confidentiality agreement, patient privacy law, or other legal obligation. I understand that no compensation is required or expected unless separately agreed to in writing. I release and hold harmless the Authorized Parties from any claims, demands, liabilities, or causes of action arising out of or related to the use of the Testimonial Materials, including claims related to privacy, publicity rights, defamation, copyright, or other intellectual property claims. I understand that once materials are published online or distributed publicly, complete withdrawal or removal may not be possible.
